Diabetes Alert Day.

Received an internal email from the matabolic research side of the company promoting Diabetes Alert Day in collaboration with the ADA. I think it totally misses the opportunity to bring aware to the different types of diabetes. As a Type 1 I get the ignorant "you got to exercise and lower your blood pressure' comments. People sometimes ignore the rather thin 140lb, 5ft 4in, human being standing in front of them.

The article you pointed to isn't news at all. Despite the constant flow of articles saying that Type 2 is caused by IR, all the gene research is finding that the common "Type 2" genes affect the ability to secrete insulin. Normal obese people just keep growing new beta cells to keep up with their weight. Many Type 2 people developed high blood sugars BEFORE obesity they just don't get diagnosed early enough for doctors to realize this. The high blood sugars cause hunger which leads to weight gain and their marginal beta cells can't handle it.

Beta cells don't have to die in Type 2--but with the average person with Type 2 having a fbg around 16-=180 mg/dl and post meal sugars of 230-300 they usually do. That's the range oral diabetes drug studies show the oral drugs make possible. Sad, eh? Especially when the only insulin docs will give most Type 2s is basal which doesn't help the post-meal numbers.

It makes sense that a Type 1 can develop insulin resistance as they get older since 90% of the regular normal population does that too. Metformin can reduce the amount of insulin a Type 1 needs in middle age though not the A1c. I think it is worth pursuing.

It's also worth noting that LADAs have a gene profile that isn't the same as Type 1 and usually includes some of the classic Type 2 insulin deficiency genes. IR might be more likely in a LADA but the condition is new enough there aren't studies to track it's natural history.

Hi Khurt, I see you also have a low A1C. My endo, at the University of Washington, Seattle WA wants me to have it higher he says that since it's an average, that 5.7 means I dip hypo to often and that has as much detrimental effect long term than does a slightly higher A1C around 6.2 or even higher. I argue with him about that because since I have started using a Navigator CGM I rarely dip hypo anymore and my A1C average has a closer range than it did when I was just testing 12 times a day and my A1C was the same but I couldn't catch the hypos and hypers as quickly.
